Becoming a Member
Enrolling Online
Enrollment is easy with NewCare MD because you have two options to enroll: online and in-person. Enrolling online takes less than 2 minutes because we only need basic contact and billing information:
- Name, Address, Email, Phone Number, Date of Birth, Gender
- Add your payment info and you’re done!
That’s It! Once you’ve enrolled, a member of our staff will reach out to you to answer any questions you may have and to schedule your first appointment to establish care.

Enrolling In Person
Enrollment is easy with NewCare MD because you have two options to enroll: online and in-person. Enrolling in-person may be a better option for you if still have questions or would like to meet our physicians first. To enroll in-person, simply schedule an Enrollment Appointment at the clinic by using the form below. When you come in we will need to collect the following basic information:
- Name, Address, Email, Phone Number, Date of Birth, Gender
- Payment Info
